Hawaii Department of Health cites two foster home operators for rule violations

The Hawai‘i Department of Health (DOH) Office of Health Care Assurance (OHCA) has issued Notices of Violation and Orders (NOVO) to two Community Care Foster Family Home (CCFFH) operators. Chita Madariaga, based in Wailuku, and Medy Madrid Simmons, located in Waiʻanae, were cited for serious violations of Hawaiʻi Administrative Rules 11-800.

According to the DOH, nursing consultants from its contractor Community Ties of America (CTA) found that Madariaga and Simmons left clients in their care with adults who were not certified caregivers. These incidents were separate and unrelated.

Both operators have been fined $500 each. They have 20 days from the date they receive the NOVO to request a hearing in writing. If no request is made within this period, the order will become final and enforceable.

The OHCA State Licensing Section certifies and regulates CCFFHs through CTA to ensure the health, safety, and welfare of individuals living in these healthcare facilities.
