Hawaii DOH fines care home operator $1,500 for forged documents

The Hawai‘i Department of Health (DOH) Office of Health Care Assurance (OHCA) has issued a Notice and Finding of Violation and Order against Romeo Valdez of Hale Lehua Care Home. The action follows enforcement related to violations of Hawaiʻi Administrative Rules 11-100.1.

According to the department, an application for an Adult Residential Care Home (ARCH) was submitted on June 18, 2025. The application included three documents identified as fraudulent, forged, or counterfeit.

A $1,500 administrative penalty has been imposed on Romeo Valdez, with each document accounting for a $500 fine.

Valdez has 20 days from receipt of the notice to request a hearing in writing. If no request is made within this period, the order will become final and enforceable. The DOH Office of Health Care Assurance’s State Licensing Section oversees licensing for Adult Residential Care Homes to ensure health, safety, and welfare standards are met for residents in healthcare facilities.

The Hawaii State Department of Health manages public health initiatives statewide through programs that include disease prevention and environmental protection efforts.
